    Eloralintide is a novel, once-weekly subcutaneous amylin receptor agonist developed by Eli Lilly for weight management. It selectively activates the amylin 1 receptor (AMY1R) with 12-fold selectivity over the calcitonin receptor in recombinant human receptor assays, distinguishing it from non-selective agonists such as cagrilintide. In recent studies, eloralintide dose-dependently reduced food intake and body weight in diet-induced obese rats, primarily through fat mass loss, while eliciting significantly less conditioned taste avoidance than cagrilintide.

    In a Phase 1 single-ascending-dose study (N=48, mean BMI 27.5 kg/m²), single doses of 4 mg and 12 mg produced mean body weight reductions of −2.5% and −4.4% at week 4, respectively. Gastrointestinal tolerability was favorable, with only 2 of 36 eloralintide-dosed subjects reporting GI-related events.

    In a Phase 2 study (N=263, mean BMI 39.1 kg/m²), 48-week administration achieved dose-dependent body weight reduction: −9% (1 mg), −12% (3 mg), −18% (6 mg), −20% (9 mg), −20% (6–9 mg), and −16% (3–9 mg), versus −0.4% for placebo. The most frequently observed events were nausea and fatigue. These findings support eloralintide as a promising candidate for weight management.
